The links on the left are for a variety of forms that you can use to help keep the club running smoothly and to help you effectively manage your team.  Read the descriptions and then follow the links below to access club forms.

Administrative Forms:

MYSA Official Match Lineup
This form is mandatory for all games.  You are required to present to the referee prior to every game.  The game cannot begin without this form....do yourself a favor and make multiple copies for your coaches folder.

MYSA Medical Release
This form is mandatory to carry with you at all times.  It is required by all sanctioned tournaments and should be used to reference emergency contact information for the players on your team.

Ejected Participant Report
Hopefully you never need this....but, be prepared.  If a player is ejected (receives a red-card) you must have the referee fill this form out after the game.


Financial & Miscellaneous Forms:

Deposit Request (into team account)
This will be used if your team has done a fundraiser, collected money from families for coaching fees, apparel, indoor training expenses or has received sponsorship funds and would like to deposit the money into your team account for disbursement at a later date.

Check Request (from team account)
This will be used if your team will register for a tournament, purchasing group apparel or participating in another team event.  Please be aware that you must have sufficient funds in your team account or the check will not be processed.  In lieu of a check, you can request that NLS make payment with a credit card, please contact treasurer to arrange this transaction.  Any variances from this policy due to a teams financial hardship will be reviewed on a case by case basis.

Club Reimbursement Request
If you incurred expenses that were pre-approved by the NLS administration, please complete this form for reimbursement.
